To correct the gummy smile, we carry out soft tissue Laser Sculpting of the gum tissues and where necessary the underlying bone can be removed to create a healthier and more attractive looking gum contours. If underlying bone tissue is removed, the procedure is called crown lengthening.
As a result of Laser sculpting, gummy smile sufferers see more of their teeth when they smile and less of their gums will be visible. We can also further improve the appearance of the teeth by using veneers or crowns.

Crown lengthening, is a common gum surgery in which gum tissue and bone is removed from around a tooth to expose more of a particular tooth. This is often carried out when a tooth needs a crown and there is not enough tooth tissue left above the gum to support the crown.
Before any crown lengthening treatment is conducted, patients will be asked to attend an initial consultation with one of our experienced dentists to review their medical history and teeth both clinically and with X-rays. Following this, the second appointment will see the patient undergo the procedure while under local anaesthetic. Intra-venous sedation is also available for any patients who wish to be sedated before having gum surgery.
The gum specialist (Periodontist) will raise the gum to pull the soft tissue away from the tooth to expose the root and surrounding bone. She will then remove some bone around the root and this will then provide adequate room to install a crown.

• First we assess your teeth to check all-on-4 suitability. We would be looking for cases where most/all of the teeth in any jaw have been lost or will soon be lost due to decay, gum disease or cases where there is a lot of bone loss.
• We will arrange for a CT scan to assess your bone quality and quantity.
• We would then remove the hopeless teeth and place a minimum of four implants in your jaw during a morning session.
• That same day we will construct a full arch fixed acrylic bridge which we will attach to your implants in the afternoon session.

Dental Implants require sufficient bone of good quality and quantity in order for the dental implants to have stability once the implants are placed in the bone.
Generally speaking the human lower jaw has tougher and more dense bone compared to the upper jaw which usually has more softer bone.
Once a tooth is removed, the bone in the tooth socket starts to remodel and resorb and after several months, there can be a lot of bone loss in the tooth socket caused by the loss of that tooth.
The ideal time for implant placement is around 6-8 weeks after tooth extraction at which time soft tissue healing has occured but a lot of bone loss has not yet occured. Waiting 6-8 weeks also allows any infection remaining in the tooth socket to be cleared up.
At Smile Architect dental clinic, we usually place our implants 6-8 weeks after extractions. At that time we inspect the bone quality and quantity and there can usually be three scenarios:
• One possibility is that there is insufficient bone present for implant placement and we will have to do bone augmentation to build-up the bone in that region. For bone augmentation we will use either donated human cadaver bone OR Bio-Oss (bovine derived) bone granules OR patients own bone.
Bone augmentation can fail (25% failure rate) and the bone graft can become infected or become loose and lead to graft failure, in which case it will need to be redone or a different treatment tried.
Once bone grafting is carried out we will have to wait around 6 months for the patients own bone to grow into it before we can place the implant.
• The second possibility is that the bone is slightly insufficient, in which case we can place the implant and do bone grafting at the same time.
We usually use Bio-Oss (bovine derived bone) along with placing the titanium implant.
• The third possibility is that there is sufficient bone present and we can place the implant without doing any bone grafting.

For a better functional and cosmetic dental implant outcome, where needed we always carry out bone grafting. We can use the patient's own bone, irradiated cadaver bone, synthetic bone or bovine derived bone. If bone grafting is not carried out in cases where it is necessary, it can lead to serious problems for the patient.
Overdentures are dentures that are supported by and attached to dental implants or root filled tooth roots, which are utilised to increase the retention and support for the overdenture.
If you wish to have an implant supported overdenture, your Pearl Dental Clinic dentist will need to ensure there is enough bone left to install the implants, which are most commonly provided in areas where traditional fixtures are less stable.
Overdentures are great for patients who wish to have the benefits of dental implants but don't necessarily wish to go through bone grafting and gum grafting surgeries that is usually required for multiple single implants or do not wish to incur the time and costs involved with having multiple dental implants.
In the case of tooth root supported overdentures, the advantage of not removing the remaining roots from the mouth is that they allow proprioception to remain and the jaw bone to remain around the roots and not to shrink away as is the case with complete dentures. The jaw bone can then be used in future for the installation of dental implants should that be required.
Alternatively to traditional fixtures, overdentures provide a large degree of stability because they are attached to and supported by the installation of dental implants in the jawbone. So overdentures have far better retention compared to complete dentures.
This means that individuals who have previously worried about their dentures falling out or becoming loose could experience an improved quality of life as a result.
Patients will not experience problems with speech and be able to eat comfortably knowing their overdentures are securely fitted following the procedure and they will not have to worry about using messy denture adhesives.
Full mouth reconstruction improves the appearance of the teeth and has a number of less obvious benefits. Full mouth reconstruction is one of the most comprehensive forms of dental treatment that is currently available.
Also known as full mouth rehabilitation or restoration, the process combines all dental specialities to individually reconstruct every damaged tooth in the mouth. This helps to correct a variety of complaints, such as improper bite position and chipped, worn down or broken teeth.
Typically, the treatment involves the application of white fillings, dental bridges, Dental Implants, crowns and veneers to multiple damaged teeth in the mouth. While it requires a huge amount of effort on the part of a team of skilled dental professionals, it is widely considered to be preferable to offering patients removable dental restorations such as dentures.

We will also provide you with a written consent form that will detail your treatment at length. The decision on which procedures are required during each full mouth reconstruction ultimately lies with the dentist and any specialists working with them on the case, although other processes may be available.
Among the potential treatments that can be used for full mouth rehabilitation are crown lengthening of the crowns of the teeth to expose a healthy and sound tooth structure, contouring of the gum tissue to bring more balance to a smile and the fitting of invisible braces to ensure the teeth are in the best possible position for restructuring.

Lingual braces are fixed onto the inner sides of the teeth. These braces have been used for many years. Their popularity is now growing as more adults are seeking orthodontic treatment. They are not seen at all so the patients who are conscious about wearing braces can get their teeth repositioned to get an aesthetic smile.
The latest lingual brackets are highly refined and low profile. Therefore, they are more comfortable for the patients. We use the Multi-Purpose Attachments for many of our patients wherever possible. These are the thinnest possible brackets available and therefore are most comfortable for our patients. All kinds of irregularities of the teeth can be treated with lingual braces. The patients are uncomfortable for a few weeks due to confined tongue space and speech problems but get adjusted later. Time required for every appointment is more. The treatment time is the same as that with the regular braces. It depends on the severity of the irregularity.
Different materials are used to manufacture tooth coloured brackets. Ceramic brackets are the most popular. Ceramic brackets with metal inserts are more advantageous to use because the teeth get corrected faster with these. Ceramic brackets are strong, not very visible, inert and stable in the oral conditions.
Invisalign / ClearPath/ Clear Aligner are appliances used for straightening teeth without the conventional fixed braces. A series of custom made trays are prepared according to the corrections required for a patient. These trays bring about changes in the positions of teeth sequentially. The patient is supposed to wear the trays as instructed and check ups are done every 6-8 weeks by the orthodontist or dentist to monitor the progress. They are not visible so adult patients not wanting to wear braces can get their teeth corrected.
They are removable so the patients do not have restrictions on food as those with the fixed appliances. As they are removable, the forces do not act on the teeth all the time. Patient cooperation is very important.
Only mild to moderate malocclusions can be treated efficiently and effectively as the control needed over teeth for complex movements is less with Invisalign/ ClearPath/ Clear Aligner as compared to that exerted by fixed braces. Patients may experience speech problems for a few days.
In many cases, it may not be possible to achieve the best possible aesthetic smile; functional and stable results without repositioning teeth with orthodontic appliances. These patients have the option of choosing appliances which are not very visible or not visible at all. Due to recent advances in orthodontics, the time taken for orthodontic corrections has reduced. In most cases aesthetic improvements can be achieved in a few weeks to a few months. Invisalign is not available in India as of now.
